IMPORTANT!
Please Read Instructions Before Completing the CGA Strategic Conference 2010 Registration and Housing Form.

  • Registration form must include credit card information when faxed or sent via the Internet.

  • Badges will be mailed to attendees who register by September 15, 2010. Badges for registrations received after that date will be held in Will Call (onsite in the registration area).

  • Achnowledgment letters confirming registration will be sent via e-mail, fax or mail to each registrant. If badge corrections or changes are needed, note them on the confirmation and return as indicated in the letter no later than September 14, 2010. After this date all changes must be made onsite.

  • A grocery retailer is defined as a store owner or operator who sells products directly to customers, sets or implements retail policies and procedures and is responsible for store conditions and profitability.

  • A grocery wholesaler is a company that buys directly from a manufacturer and sells to retailers. (This registration rate is limited to full-line grocery wholesalers only.)

HOUSING INFORMATION
The CGA Strategic Conference has reserved accommodations at the Mandalay Bay Resort and Casino and will secure reservations for registrants requesting accommodations prior to September 15, 2010. Please register early in order to secure your hotel, as space is limited. After September 15, 2010, room rates and availability cannot be guaranteed.


PAYMENT
Payment in U.S. funds must accompany this registration in order to be processed. VISA, MasterCard and American Express are accepted.


CANCELLATIONS
To receive a refund for payment of registration, notification must be received in writing no later than September 15, 2010. Registrations received after this date are nonrefundable, but substitutions will be allowed onsite.

Mandalay Bay Resort and Casino is enforcing a 72-hour cancellation policy. Cancellation of your reservation within 72 hours of your reservation will result in a penalty charge equivalent to one night's room and tax charges.

    October 19 - $165

    Credit card information must be completed for hotel guarantee. Your credit card will be charged one night's room fee, plus 12% tax, at the time of registration. If paying for hotel reservation by check, check must be made payable to Mandalay Bay Resort & Casino for the full amount including the 12% tax and must be received by the hotel two weeks prior to arrival. Please contact CGA staff for assistance.

    To change or cancel hotel reservations prior to October 8, 2010, note them on the confirmation as indicated in the letter and return to CGA. Changes will be made on a space available basis. After October 8, please contact the Mandalay Bay Resort and Casino directly at (888) 632-7000.

    Hospitality suites and function space is limited at Mandalay Bay Resort and Casino and will be reserved on a first-come, first-served basis. For suite rates and availability, please call (916) 448-3545. Requests are subject to approval from CGA Show Management. It is our policy that Conference attendees are not allowed to sponsor any functions that conflict with a scheduled Conference event. This applies to events held in hotel suites, meeting rooms and at any outside location.

    All registrations include: General Session, Round Table Discussions, Exposition, Breakfast & Lunch hosted by The Illuminators, and Sunday's Opening Reception.
